Optional Tool to Help Businesses Open – Maryland Department of Health (MDH) Employee Health Screening form. See attached. While you do not have to use THIS tool, you should use a comparable tool to assess employee exposure risk.  Clearly communicate expectations to your employees and document the results of completed assessments prior to shift starts.  For employees working extended or double shifts, an assessment should be performed every 8 hours.
